Unfortunately, during server shutdown, this might never happen again. Other option would be "not to handle" the issue in the transport layer. That would trigger the error handler logic, which would also need the session state to be logged in again. Best option, for such case, is to tell upper layers that the command was handled during the transport layer error handler helper, marking it as DID_NO_CONNECT, which will allow completion and inform about the problem. After the session was marked as ISCSI_STATE_FAILED, due to the first timeout during the server shutdown phase, all subsequent cmds will fail to be queued, allowing upper logic to fail faster. [ Upstream commit 934140a ] cachefiles_read_waiter() has the right to access a 'monitor' object by virtue of being called under the waitqueue lock for one of the pages in its purview. However, it has no ref on that monitor object or on the associated operation. What it is allowed to do is to move the monitor object to the operation's to_do list, but once it drops the work_lock, it's actually no longer permitted to access that object. However, it is trying to enqueue the retrieval operation for processing - but it can only do this via a pointer in the monitor object, something it shouldn't be doing. If it doesn't enqueue the operation, the operation may not get processed. If the order is flipped so that the enqueue is first, then it's possible for the work processor to look at the to_do list before the monitor is enqueued upon it. Fix this by getting a ref on the operation so that we can trust that it will still be there once we've added the monitor to the to_do list and dropped the work_lock. The op can then be enqueued after the lock is dropped. The bug can manifest in one of a couple of ways. We should just move pwmchip_sysfs_unexport() where it belongs, which is right after pwmchip_sysfs_unexport_children(). In that case, we do not need separate functions anymore either. We also really want to remove sysfs irrespective of whether or not the chip will be removed as a result of pwmchip_remove(). We can only assume that the driver will be gone after that, so we shouldn't leave any dangling sysfs files around. This warning disappears if we move pwmchip_sysfs_unexport() to the top of pwmchip_remove(), pwmchip_sysfs_unexport_children(). That way it is also outside of the pwm_lock section, which indeed doesn't seem to be needed. Moving the pwmchip_sysfs_export() call outside of that section also seems fine and it'd be perfectly symmetric with pwmchip_remove() again. So, this patch fixes them. commit cf3591e upstream. Revert the commit bd293d0. The proper fix has been made available with commit d0a255e ("loop: set PF_MEMALLOC_NOIO for the worker thread"). Note that the fix offered by commit bd293d0 doesn't really prevent the deadlock from occuring - if we look at the stacktrace reported by Junxiao Bi, we see that it hangs in bit_wait_io and not on the mutex - i.e. it has already successfully taken the mutex. Changing the mutex from mutex_lock to mutex_trylock won't help with deadlocks that happen afterwards. [ Upstream commit 76e7527 ] Some servers seem to accept connections while booting but never send the SMBNegotiate response neither close the connection, causing all processes accessing the share hang on uninterruptible sleep state. This happens when the cifs_demultiplex_thread detects the server is unresponsive so releases the socket and start trying to reconnect. At some point, the faulty server will accept the socket and the TCP status will be set to NeedNegotiate. The first issued command accessing the share will start the negotiation (pid 5828 below), but the response will never arrive so other commands will be blocked waiting on the mutex (pid 55352). This patch checks for unresponsive servers also on the negotiate stage releasing the socket and reconnecting if the response is not received and checking again the tcp state when the mutex is acquired. Furthermore, item 183 is the last item in the leaf. btrfs_log_prealloc_extents() is responsible for logging prealloc extents beyond i_size. It first truncates any previously logged prealloc extents that start beyond i_size. Then, it walks the filesystem tree and copies the prealloc extent items to the log tree. If it hits the end of a leaf, then it calls btrfs_next_leaf(), which unlocks the tree and does another search. However, while the filesystem tree is unlocked, an ordered extent completion may modify the tree. In particular, it may insert an extent item that overlaps with an extent item that was already copied to the log tree. This may manifest in several ways depending on the exact scenario, including an EEXIST error that is silently translated to a full sync, overlapping items in the log tree, or this crash. This particular crash is triggered by the following sequence of events: - Initially, the file has i_size=4k, a regular extent from 0-4k, and a prealloc extent beyond i_size from 4k-12k. The prealloc extent item is the last item in its B-tree leaf. - The file is fsync'd, which copies its inode item and both extent items to the log tree. - An xattr is set on the file, which sets the BTRFS_INODE_COPY_EVERYTHING flag. - The range 4k-8k in the file is written using direct I/O. i_size is extended to 8k, but the ordered extent is still in flight. - The file is fsync'd. Since BTRFS_INODE_COPY_EVERYTHING is set, this calls copy_inode_items_to_log(), which calls btrfs_log_prealloc_extents(). - btrfs_log_prealloc_extents() finds the 4k-12k prealloc extent in the filesystem tree. Since it starts before i_size, it skips it. Since it is the last item in its B-tree leaf, it calls btrfs_next_leaf(). - btrfs_next_leaf() unlocks the path. - The ordered extent completion runs, which converts the 4k-8k part of the prealloc extent to written and inserts the remaining prealloc part from 8k-12k. - btrfs_next_leaf() does a search and finds the new prealloc extent 8k-12k. - btrfs_log_prealloc_extents() copies the 8k-12k prealloc extent into the log tree. Note that it overlaps with the 4k-12k prealloc extent that was copied to the log tree by the first fsync. - fsync calls btrfs_log_changed_extents(), which tries to log the 4k-8k extent that was written. - This tries to drop the range 4k-8k in the log tree, which requires adjusting the start of the 4k-12k prealloc extent in the log tree to 8k. - btrfs_set_item_key_safe() sees that there is already an extent starting at 8k in the log tree and calls BUG(). Fix this by detecting when we're about to insert an overlapping file extent item in the log tree and truncating the part that would overlap. The code in ocfs2_dio_end_io_write() estimates number of necessary transaction credits using ocfs2_calc_extend_credits(). This however does not take into account that the IO could be arbitrarily large and can contain arbitrary number of extents. Extent tree manipulations do often extend the current transaction but not in all of the cases. For example if we have only single block extents in the tree, ocfs2_mark_extent_written() will end up calling ocfs2_replace_extent_rec() all the time and we will never extend the current transaction and eventually exhaust all the transaction credits if the IO contains many single block extents. Once that happens a WARN_ON(jbd2_handle_buffer_credits(handle) <= 0) is triggered in jbd2_journal_dirty_metadata() and subsequently OCFS2 aborts in response to this error. This was actually triggered by one of our customers on a heavily fragmented OCFS2 filesystem. To fix the issue make sure the transaction always has enough credits for one extent insert before each call of ocfs2_mark_extent_written(). commit b684c09 upstream. ppc_save_regs() skips one stack frame while saving the CPU register states. Instead of saving current R1, it pulls the previous stack frame pointer. When vmcores caused by direct panic call (such as `echo c > /proc/sysrq-trigger`), are debugged with gdb, gdb fails to show the backtrace correctly. On further analysis, it was found that it was because of mismatch between r1 and NIP. GDB uses NIP to get current function symbol and uses corresponding debug info of that function to unwind previous frames, but due to the mismatching r1 and NIP, the unwinding does not work, and it fails to unwind to the 2nd frame and hence does not show the backtrace. Heiner reported a lockdep splat [1] This is caused by attempting GFP_KERNEL allocation while RCU lock is held and BH blocked. We believe that addrconf_verify_rtnl() could run for a long period, so instead of using GFP_ATOMIC here as Ido suggested, we should break the critical section and restart it after the allocation. Finally, invoking device_attach_pasid_table() causes the issue. `git bisect` points to the offending commit 474dd1c ("iommu/vt-d: Fix clearing real DMA device's scalable-mode context entries"), which releases the pasid table if the device is not the subdevice by checking the returned status of dev_is_real_dma_subdevice(). Reverting the offending commit can work around the issue. The solution is to prevent from allocating pasid table if those devices are subdevices of the VMD device. The problem is encoutered with tests added by commit 38608ee ("bpf, tests: Add load store test case for tail call") This happens because tail call is done to a BPF prog with a different stack_depth. At the time being, the stack is kept as is when the caller tail calls its callee. But at exit, the callee restores the stack based on its own properties. Therefore here, at each run, r1 is erroneously increased by 32 - 16 = 16 bytes. This was done that way in order to pass the tail call count from caller to callee through the stack. As powerpc32 doesn't have a red zone in the stack, it was necessary the maintain the stack as is for the tail call. But it was not anticipated that the BPF frame size could be different. Let's take a new approach. Use register r4 to carry the tail call count during the tail call, and save it into the stack at function entry if required. This means the input parameter must be in r3, which is more correct as it is a 32 bits parameter, then tail call better match with normal BPF function entry, the down side being that we move that input parameter back and forth between r3 and r4. That can be optimised later. Doing that also has the advantage of maximising the common parts between tail calls and a normal function exit.
