Don't have a means of fully testing this at the moment myself, but if you believe this PR is finished, we can merge it in and make an RC1 release to allow you and others easier testing of the new features and upgrade testing. I can change PR #71 to have the version be 1.4.0rc1 and go from there. Can actually make the extension version be rc1 as well so if anyone installs it they can also upgrade directly to whatever the final 1.4.0 release will be.

This commit gets rid of the deprecated GUCs mechanism, thereby rendering
the old `set_user.superuser_whitelist` GUC useless.

Users should update their postgresql.conf to use the newer
`superuser_allowlist`.
