This is the fifth PR in an attempt to break my work on 4.0 into reviewable chunks.
This pull request migrates extensions to the new architecture.
Because the architecture will evolve further in the subsequent PRs, this is not necessarily the best example to look at as an example of how to migrate an extension, but overall it gives an example of the sort of changes that are required.
