Fix DoctorAddPatientInfo View

• "Blood Pressure" is not loading/showing properly (always 0/0)
• "Reason for Visit" does not load/is empty
• "Known Allergies" display height
• "Patient Past History" does not load/show properly (it does exist in database as the nurse can see the updated patient past history)
• "Contact Patient" shows a "phone number" field that should be taken out
• Clicking "Done" button takes you to template DoctorPickPatient instead of the one of the current doctor logged in
• "Past Tests" tab has tests move upwards instead of downwards, making them go out of the upper margin
