Experimental Mapnik input plugin for PostGIS Raster
See mapnik/mapnik#1660

The plugin is a copy of the GDAL plugin but forces use
of the PostGIS Raster plugin of GDAL an uses PostGIS-like
parameters of Mapnik.

Differences with "postgis" plugin:

 - "geometry_field" is replaced by "raster_field"
 - Datasource "type" is "pgraster"
