None of the icons that the default ruleset, rules.osm, refers to are anywhere in the repo.

Please check the folder /usr/local/share/smrender/SeaMapSymbols.
You can symlink it to the folder from where you run Smrender.
ln -s /usr/local/share/smrender/SeaMapSymbols (execute from within the folder where you run smrender)

I suggest to use the ruleset found in /usr/local/share/smrender/r100k_ye as a good starting point (rules.osm is pretty old).

Well, it depends on what you consider as default. But since you have to setup the rendering directory before the 1st time you run smrender on your data, it's up to you to call your ruleset rules.osm. And this does not necessary have to be a file, it could be a directory as well.
Have a look at Chapter 9 "Usage Examples" of the documentation.

And of course you can use the option -r the specifiy your ruleset.
