个人认为不加空格为好，且引用公证文件名称应用书名号。
另外对究竟是「知情/同意」还是「知情同意」持保留意见，但既然后文有提到根据医院可能有所不同，我认为这里具体是哪个并不重要。

《性别重置技术临床应用管理规范（2022 年版）》要求是「已告知」，我个人已知目前大陆大医院均仍为「同意」
