This was an unformatted version of How_To_Create_Lasting_Change.asciidoc which became ch04_Create_Real_and_Lasting_Change.asciidoc below

This really helped me be able to actually read the sections people wrote, even if it's not really a finished book.
Renaming all the files made it clear which ones weren't being referenced in book.asciidoc and were missing from the PDF I built.
