Add/update tumor types and diagnosis

I am doing an exercise with AI-assisted harmonization and realize some deficits in the model that might be encountered / cases of non-optimal mappings when we're missing target terms that should be there. I think we should add:

• Vestibular Schwannoma - https://ncit.nci.nih.gov/ncitbrowser/ConceptReport.jsp?dictionary=NCI_Thesaurus&code=C3276
• Add "Unknown" to diagnosis.

And make a note that

• Localized Neurofibroma is deprecated and real preferred term Cutaneous Neurofibroma should be used -- I think we are keeping this for backwards compatibility with JHU Biobank (?), but having it available as an option is confusing
