No consistency between title in admin panel and in user account

Editing a title in the user section of the admin panel does not affect the title in the account section. Also, the titles don't match (the account section has 'Doctor' [it should be 'Dr' anyway], where the admin panel doesn't). Generally titles need to be reconsidered as they're very inconsistent and buggy.

• Titles need a centralised collection to be stored in. Would be helpful if this was translatable.
• Changing title on the front-end doesn't change the back-end, and does not update the DB
• Changing title on the back-end doesn't change the front-end, and does update the DB
• Where is the front-end reading the current titles from?
