fix(input): camel-case md-prefix and md-suffix (angular#2639)

* Renames the md-prefix and md-suffix directives to camelcase.

Closes angular#2636

BREAKING CHANGE: md-prefix and md-suffix are now mdPrefix and mdSuffix
