I am not sure this is a valid issue, looking at the examples in those cases
it seems clear from the angle of the arrows to which way the oneway belongs
(at least for me). It might generally be better in the case of dual
carriageways becoming a single carriageway, if this transition was made
faster (more explicit) than in the examples, i.e. don't let them converge
slowly but make a distinct statement at the point they split and put the
osm way "ASAP" at the centre of the road. E.g. here:
http://www.openstreetmap.org/?mlat=41.81902&mlon=12.39761#map=19/41.81902/12.39761

But this is maybe a question of style / taste.

In case of doubt, for practical reasons, zooming the map in would usually
disambiguate.

It is impractical for exporting map to image (at least form me it is typical use of OSM).

According to my testing it is solved by #1840
