Problem description

The icon in the toolbar in full-screen dialogs is not vertically aligned with the title:

First Header	Second Header	Second Header

Steps to reproduce the problem

This form displays two first dialogs visible above:
identify-user-audit.xml.txt
The third dialog is select_one/multiple minimal available in the All widgets form. It doesn't contain any title but still it's visible that the icon is not centered.

Expected behavior

The icon should be aligned with the title.
