This PR fixes the problems in 1.4.0, #100 and #101. It also fixes #97 and #98.
There's a lot here, and I'd probably make it 1.5.0 except that 1.4.0 was so completely broken that it seems reasonable just to call this 1.4.1. There are upgrades on new versions of XML Resolver and Saxon XInclude processor, and the build system has been reworked quite a bit.
