Afform - Fix button appearance and block form during submission

Overview

UX improvements to the Afform submit button

Before

Button did not get correct classes to be styled with bootstrap.
No visual feedback during form submission.

After

Form "blocked" (disabled) during submission with visual feedback to the user.
