Add form-action directive to default CSP header. Limit form target to 'self'.

Update CSP directive setting used in Docker to match the installation default settings.
Update default from 'Content-Security-Policy-Report-Only' to 'Content-Security-Policy'. This will cause the CSP directives to be enforced.
