MultiDict issues in http request

1. Why MultiDict is used for request headers, not CaseInsensitiveMultiDict?
2. Why getall() neither returns empty list when there is no value, nor have default argument?

I don't like caseinsensitivemultidict, but can live with it untile we introduce ServerRequest, everything else same as Andrew—
