Add a pull request template so community contributions can meet our expectations for completeness.
This may be temporary - I have checked with Orb to see if the timeframe parameters being required is an intentional change.

Also fix CI rot issues caused by changed API behavior, and try to address the issue where upgrading rust versions causes test matrix leakage across PRs.
