Replaced "Version" by "App Version"

I think the examples in the templates are enough to clarify it but I'll merge this. If you want, you can update it in other repos too. |

Having found that several people have misinterpreted the "version" field of the issue template and indicated the version of their OS, I propose to rename this field "app version".
