Remove headers
dyna_pthread.handdyna_svga.hthat were not included in any other file.dyna_pthread.hin linux/lib was a duplicate ofdyna_pthread.hin lnxmvelib/, and never included anywhere.This also removes the DECLARE_POINTERS define that was exclusively used in these 2 headers, and
dyna_curses.h, edited to get rid of it as well.
