**Why**: The license address may not be valid or the latest address for the user. Moreover decoupling address from driver's license info will allow us to support passports. **How**: Add a new summary information screen which will allow the user to validate the information they want to submit for proofing. Allow the user to change the social security number or the address. Add a new optional address form to collect address information if it is different from the driver's license.
