On QA, unable to add second author with affiliation

NOTE: Affiliations are currently disabled due to form complexity. Until we re-enable affiliation entry and/or remove re-form, this is on hold and may go away when the feature is working correctly.

First author with affiliation works just fine, but I can't add a second author with an affiliation.

1. Create new item.
2. Add author https://sandbox.orcid.org/0000-0003-3437-349X, update name to "Justin Littman" and use ROR lookup to add "Stanford University" affiliation.
3. Deposit item.
4. Edit item
5. In what's changing type "adding author Create landing page #3"
6. Add author https://sandbox.orcid.org/0000-0002-5466-7797, update name to "Vivian Wong" and use ROR lookup to add "Stanford University" affiliation.
7. Deposit item.

I tried this using a different affiliation for the second author ("Berkeley College") and the result was the same.

I tried this a third time where I did not put in an affiliation for the first author but DID put in the "Stanford University" affiliation for the second author. This deposited but the affiliation was not saved.
