I think we've only had one other report of an issue with duplicate emails, but we didn't have access to debug directly and weren't able to replicate it ourselves on our installation.
After you do that leave a comment (and subscribe to notifications), then leave a reply to that comment and check how many email requests have been logged for that notification email.
That's the first step, to see if it's easy to replicate the issue on your site.

I've had reports of users receiving multiple duplicate emails when someone responds to their comment.
