Ability to create schedules from "add sched history record" dialog

The current "add sched history record" dialog allows the user to choose an existing schedule by a lookup dform (i.e., she must know the exact schedule ID or code). This is not flexible enough. Add miniMenu options:

1. choose an existing schedule by browsing
2. enter a new "boilerplate" schedule (with identical intervals Mon-Fri)
3. enter a new "custom" schedule

These would leverage existing features but instead of returning to the schedule menu on completion, the user would be returned to the "add sched history record" dialog with the fields filled out.
