Enhance "Make a suggestion" form so the user can reference the page they clicked through from #1026

There are links at the top of every page on the Docs Hub to "Edit this page" and "Make a suggestion." "Make a suggestion" takes the user to a Documentation Feedback form on the PKP website. A user pointed out that it would be easier to make the suggestion if they could still see the page or document they had linked from. They suggested having the name of the document appear on the form, but I think it would be better if the form opened as a pop-up on the document itself, so the user could see the page or doc while they make the suggestion, or something similar.
