Combine the Problem Results and Problem Attempts tables on the Course and At-Risk Learner dashboards

User Story
As a course delivery team member, I want the two separate problem and attempts tables on the Course and At-Risk Learner dashboards combined, so that there are less tables to sift though when looking for data on these dashboards.

Acceptance Criteria
Given I have access to the Course and At-Risk Learner dashboards
When I view the Problem Engagement information on the Course and At-Risk Learner dashboard
Then I see that the Problem Results and Problem Attempts tables are now combined (with repetitive information such as the problem name, link, and location information only appears once)
