Weird counting and ordering

Something weird is happening with the current Raadstalk, the ordering is not correct. Below here, 'concept' is exactly 10000. Could it be the maximum number Elastic is showing? In that case "track_total_hits": true gives the accurate counts.

Also, some items are duplicate but this might be resolved by a new import.
