In the top level maildir we could have a file "pull" that descirbed where to pull from for that maildir (whether IMAP or SSH or some other scheme).
Pull commands can the check for that file on whether to actually try a pull or not.
maildir/pull can be rewritten to read the pull details from the file instead of from customize, this means we can get IMAP in there.

maildir-make needs to be cleaned up to allow pull details to be recorded when a top level maildir is made.
