Give more guidance on describing what has changed

Stepping back a bit, we think this document may be trying to put too much weight into a single tag. There is no substitute to reading the actual document and see what it says is being updated, amended, extended, and referenced.

A bigger problem here it is that we are not consistent with describing what is changed in a document. This isn’t fixed by new tags. All these tags do is to say read the document.
