In the explorer tree, I was trying to add a .gitignore file. At the same time, a process was running which was adding / deleting files in the same folder. Each time a file was added / removed the explorer tree lost focus. So it was impossible to add the new file, since before I could "right click", "new file" then enter the new name a temp file addition / deletion took focus away and the new file vanished.

I don't think this is a duplicate - it may be that a fix for #71315 also fixes this though.
