Issue Type: Bug

Perhaps a duplicate of #48258

Steps to reproduce:

  1. Create file.mjs to contain an error, and open in VSC
  2. Rename to FILE.mjs
  3. Run the file in VSC terminal
  4. Access the hyperlinked line in the stack trace
  5. You now have two copies of the file open, with different name.s

I'm on Windows.....
