Fix locked upsert on device_lists_remote_cache #4132

We don't need to do a locked upsert on device_lists_remote_cache or device_lists_remote_extremities, because the changes are protected by an application-level linearizer in DeviceHandler._handle_device_updates.

While we're in the area, let's fix the non-uniqueness of the indexes on these tables.

Fixes #4097.

We can rely on the application-level per-user linearizer.
The indexes on device_lists_remote_extremeties can be unique, and they
therefore should, to ensure that the db remains consistent.
