Normalize plugin name #33

Right now, the plugin is referred to as both "WooCommerce Order Tables" and "WooCommerce Custom Order Tables". This usage should be normalized ahead of an official 1.0.0 release.

Discussed this with Chris, will normalize on "WooCommerce Custom Orders Table" as the plugin name, and "woocommerce-custom-orders-table" as the slug/text-domain.

Update: "Orders", not "Order"

Update 2: The hyphen will only be used in the plugin header (e.g. "WooCommerce - Custom Orders Table"), as it's a common convention when releasing a plugin that extends another plugin (in this case, WooCommerce)
