This does not appear to be working on our TFS 2015 Update 2.1 system. What is the minimum level?

I've tested the latest release (of today) with TFS 2018 and that seems to work. Can you test again and maybe share why it's not working?

I just installed the latest version of Sprint Goal. It is still not working for me. I am now on TFS 2015 Update 4 (on prem). Same issue, the Goal does not show up on the Sprint Iteration backlog page.

If you open the developer tools of your browser, does the logging section provide some additional information? And if not, what do you see in the network panel, are requests being blocked?

The only thing that was logged was this.
