Add rurality?

Hi Lee.

Would it be possible to add a measure of rurality to the tool as a county-level characteristic? I'm attaching what SEER uses. I think we could use the 2013 version. What do you think?

I'm also wondering about persistent poverty counties.

Neither of these would have distribution based quantiles. Instead, rurality is a 9 level variable and persistent poverty is binary. would these be difficult to incorporate?
