Cannot add roles while creating an app within INT Portal

Current Behavior

While creating an app in the INT portal it is required to add user roles by the provided template.
After adding the values I am unable to continue as they are not successfully added to the form.

Expected Behavior

Successfully uploaded user roles are added to the form to be able to continue creating an app.

Steps To Reproduce

1. Create an app
2. Proceed to step "Technical Integration":
3. Click add file
4. Choose attached users.csv file
   users.csv
5. Click upload app roles
   
   

Result: As the roles have not been loaded the save button will lead to an error message.
