The checkbox on the create an event form needs to be elaborated on for clarity. Maybe a popover that explains what fields are in the survey.

I'm reasonably confused about what the survey is / how to create an event / how to find the survey.

When you register for an event, it's the part that asks questions like "how did you find out about this event?" and "would you like to receive promotional materials for this event?" - could hypothetically include more things in the future.

It's one of the last things you get to while registering.
