Improve field names for default annotations

The annotation field names need to be made nicer, and more descriptive -- especially for default annotations. The following images shows what the end result should look like, in annotation and annotation_format tables. Of course, these changes need to be made in the relevant annotation.xml files which we host online. After testing this change, we can manually apply the changes by copying these tables to existing databases (circumventing the need to reinstall and reapply annotations).
