assigning a person to Unified Command role, then switching to Incident Commander mode, then Back to Unified Command mode, Unified Command role reverts to vacant.

same for any roles created under unified command 1, 2 or 3 (the roles themselves disappear, not just becoming vacant)

I've done some updates on this. If the UC2 or UC3 roles are filled, or have any subordinate roles that are filled, the system will prompt you and ask if you want them all cleared. If you say yes, it clears all assignments and removes those two roles along with all subordinates.
If you say no, it goes back to unified command.

Please confirm this solves the issues?

fresh incident, give name a
org chart, swap to unified command
make role under unified command
swap back to incident commander mode

Crash to desktop
