(doc) Update changelogs to point to release notes

Rather than duplicate the information in multiple places, which can
result in inconsistences (as happened here where the licensed changes
hadn't been updated for several releaes), let's keep this information in
one place.

That place will be the docs repository, and everything should direct
folks to there.
