Visually indicate EEO fields required when enabled

Current behavior

When eeoc options are enabled, they are required by default, but there are no visual indications of the fields being required

Expected behavior

If a field is required I should see an indication that it is required

Steps to replicate behavior

Enable eeoc options in app.json
