Expand the error logging for runs that fail in carrot

Since we added the carrot_failed status for runs and the run_error table, we have not made sure all the ways in which errors in carrot can cause run failures have run_error logging. It would be good for someone to go through the code and find all the places where run's can have their status changed to carrot_failed and make sure we're logging failure reasons in the run_error table.
