AppVeyor: Build with VS 2019, Cuda 11 #776

  • Updates the build image to and builds with Visual Studio 2019
  • Builds with CUDA 11
  • Minor cleanup/refactor

Succeeds #775

Currently Visual Studio 2019 outputs over 4000 warnings, which can be seen on this AppVeyor build or in the logs below.

@dsmtE Thanks for fixing the VS 2019 builds (see #778) in #805! AppVeyor builds passes now.
