(docs): update docs to describe this version

- changelog-maker -> @agilgur5/changelog-maker
- don't say "to CHANGELOG.md" because it only outputs the changelog
  for a single release, not for the entirety of the repo
- describe and show example of the new format
- remove license section from README.md as LICENSE.md covers it
