Mute patients on combination of person and clinic not just patient

Currently the mute filter is turned off, but if we want to enable this again it might be better to not just mute on patient but on patient-clinic combination, so a patient is not called repeatedly for the same clinic, but they are called for another clinic.

This is especially important for clinics like sport and revalidation which treats mostly patients that were referred from other clinics
