I noticed that

registry-config-base/ldregistry/templates/actions/create-register-page.vm

Line 69 in 69f3e4c

<select class="form-control" name="$tag.formField" id="$tagindex-input">
is outputting HTML <select> elements with id="$tagindex-input" and that the corresponding <label> elements point to this ID.
