Messages are sent for all prescribed medications when only one is changed

Description

When a medication is either changed or added to a patient's dashboard, they will receive home screen notifications for all listed prescribed medications saying that they were all changed.

Reproduction

Start with a user that has at least 2 medications listed as taking and change one of them in the dashboard. You should see messages on the patient's app for each medication they are taking that the dosage has changed.

Expected behavior

The patient should only receive a message that the dosage has changed for just the one medication that was actually changed.
