Tint actor names in combat tracker according to health

As an extension of the existing module functionality, consider tinting actor names in the combat tracker according to health as an alternative to/in addition to the current hover over display.

This could also potentially also be taken further and the damage state string could be appended to the actor name string in the combat tracker HTML.
