Creating actions with toolbar sometimes fails

If this affects the front-end, screenshots would be of great help.

User showed this to me on their screen. They first claimed they were unable to create an action with both a link href and selector. Then they showed this and indeed it didn't work. However, they then didn't manage to create an action even with the link href off anyway. They had previously created actions with the toolbar with no problems though.

An empty action then appeared on their list.

Expected behavior

Creating an action should work.

How to reproduce

I'm unable to reproduce, but this is what I saw live on the user's screen:

1. Click to create new action on the toolbar
2. Select an element with a link
3. Click to save the action
4. Nothing happens
