Reproduction:

Going into the medical delivery shute on MetaStation when the exit was destroyed deleted my body as I reached the end of the pipe. It gave me the "you might be able to struggle free" and then I was completely dead. Testing with admin tools had this be a recurring issue, as this happened every time.
